Sad and Scarry.

Anyone know the circumstances on just how he was electrocuted? I work on these systems and its a need to know for me.

I was really hoping someone had an answer for me here. I guess no one connected to PW knows what happened.
I can only speculate that the pump motor was energized and not grounded.
I will always do my inspections with the power off first.

Mainland elec. engineer....seems there was no lockout procedure followed (unfortunately we have found this to be very common here....one of the hardest things for us to get used to....coming from an environment that enforced multiple lock lockouts)

True that. I've been shocked by someone turning on a circuit that I was working on. Now if I'm on a job with other people working I will disconnect the circuit from the breaker.

Where is OSHA???

The lack of, refusal to, or ignorance of lock out/tag out is a willful OSHA citation!

OSHA In Puna. Right. I was on a big commercial job for over a year on Maui and OSHA officials showed up only once. They shut the entire operation down for a day and then we never saw them again.

Agreed that the counting of OSHA or HIOSH here is like counting on a month of no rain here in Hilo, but why can't the name of the contractors involved in this accident be named?

I have seen jobsites in the mainland that have had the local Sheriff arrest people who circumvented lock out tag out procedures.

Here, I guess a few deaths a year is OK.

But I would sure like to see the names of the Contractors/County/State entities involved in this one!
