Are you talking about a propane gas on demand water heater?
It may be due to improper adjustments and or this can also happen if you turn your water flow to a low setting effectively inducing the water to over heat in the system because the flow is too low at the heat exchanger. Try turning up the water flow higher and or increasing the faucet temperature of the water you use.
I've experienced the same problem before and it was due to having the water flow to low, I eventually had it adjusted to meet my preferences.
