Example of the ACA "not working as intended": while insurers can't charge an individual for more than $6350/year out-of-pocket, they also don't have to cover expensive drugs if they don't want to:

I thought the idea of mandatory insurance was that the risk pool would become large enough that it wouldn't be necessary to charge "certain people" more for the increased expenses associated with their poor health. At least this is how it was sold to the public.
