Most states allow as-built permitting for the leeway of creative open plan building.
How often do folks in Hawaii - Build First intentionally to then immediately permit after as an as-built? I know there's a doubled fee applied to the permit cost in this scenario although how about electrical and plumbing too?
Does it go through the electrical and plumbing permitting too?
Can a septic permit be issued without a building permit?

A couple of things IMHO happen, people get a permit and then make changes, submitting all changes at the end... which theoretically makes those changes an an "as-built". The second thing is the changes are done after the final, and never permitted.
Electrical and plumbing can have a penalty also. This is kind of an "iffy" item - if you get your changes (inc any plumbing and elec changes) in before plumbing and elec final - then they dont "appear" to be charging double permit fees for those.
Waste water permits are issued during the permitting process. But because the waste water system is designed on the # of bedrooms etc - a change in design may necessitate a change in waste water size unless you planned ahead. For example, you have a studio but know that in a few years you will add on and make it a 2 bedroom. Then you need to make waste water design suitable for a 2 bedroom in the beginning , or larger.
On waste water systems, it appears to be that you can not permit a system without a building permit application because the DOH wants to see the plot plan and floor plan. But it does happen where a plan is submitted by CE and approved, and then house is never built but waste water is in already and inspected.

I understand the need for the plot and floor plans, but does that mean you need to have a building permit in hand to be granted the septic permit at the DOH?
When a home is built and completed without permits; owner then applies for permits, are double permit fees applied to each permit type respectively?

To the best of my knowledge the answer is yes. I am aware of no circumstances where the CoH or would permit a septic without a concurrent house plan application which includes a plot plan.
I do not know the answer to question two. Best to just call the building department.
Assume the best and ask questions.

961 8331 call them on Monday. Call them on Tuesday for a different ans. Call Wed and talk to someone else for a third ans.
The only way so far I have gotten a clear answer is to say "okay so you [ the inspector ] are saying this and this" so I am going to quote you as of xx:xx on x/x/xx and email my client and I read statement back to them. Then they went back and checked. Came back with a different answer.
Dont get me wrong - the staff at Bldg Dept are all nice, and help as much as possible. This stuff drives me crazy.

So if people build without a permit, they're building without a waste water system?

Regarding electrical, for an as built they charge you a penalty, $100.00 or double the permit fee, whichever is more.
Before the final electrical inspection you can file a supplemental permit based on the original permit to reflect any additional circuits or feeders. No penalty here, just regular permit fees--which I suspect will go up a lot in the near future.
