LOL I had NONE of that stuff. I've never been a good record keeper and living where we did last it was never important.

I went to register my daughter and they said I needed that OR an appointment to get shots.

I called the last school she was in, they printed off her records and faxed them to the school. My daughters doc that gave all her childhood shots has passed away and I have no idea how to even get those records.

Soooooo

We took that fax to our local doctor and they signed off on everything since the school in Alaska had already verified that the information was correct.
