Facility fees charged by medical service providers

You normally see them when using a hospital, but other providers such as doctors practices, clinics and surgery centers are now starting to charge them. Problem is some insurance won’t pay the fees and you’re on the hook.
Has anyone encountered this?

When I took my wife to the Keaau Urgent Care, one of the items on the bill was "Urgent Care Fee". I think it was $50.00. It seemed like a scam to me so I argued with the girl over it because it was an URGENT CARE clinic and it seemed like some bullsh!t double billing. She told me to sit down and shut up, so I paid it, and forgot about it. SEVERAL months later, we received a check from them for $50.00. The only explanation I can think of was that the fee was illegitimate and they got called out on it and sent refunds rather than risk class-action litigation.

Maybe "facility fee" is the illegitimate flavor of the day?
