Looks like you have been doing a lot of the right things for her so far!
I think I have found some more valuable resource of information for you dog.
First off it is good that you declined the Prednisone according to this site which I have found to be great with their knowledge and expertise. (I too do not want to touch the drug if I do not have to. It is a very powerful drug.)

She is so young and it is hard to see them with this kind of thing happening to them. The good news is you might try these things, you can try another round with the Ivermectin, which can be problematic if given at a dose that does not work for her (also can give it by injection to get the correct dosaage or on a piece of bread) I don't know how much you have to give though if that would work if it is a large amount. or since it is does not have a palatible taste, she may not have recieved enough to get rid of the parasite, so you ight have to increase the dosage for the next treatment. or just go with a topical product called interceptor. It claims to get rid of demodectic mange in three months time.

the other thing to remember is since her immune system is lower for resitance with this, you might want to worm her with a differnt product and one that kills whipworms, since many do not do so. Here is a chart of the drugs that do kill whips also.
My dog was real debilitated by them and since they do not always show up in a fecal sample they can bypass you knowing that they have them. At any rate, you want to rid her of worms since they are so easy to get again and again.

It also says that this usually will resolve in a puppy and not carry into adult hood. But to not breed these animals because it can be linked that way. Not that you were going to, but your breeder whom you got her from needs to know this.
Thank you for the website. Are they real reliable and send it right out? I have found Frontine at a few places that have that price. One thing I do is chart out the dogs weights and give the needed dosage not a while capsule. Since I do not want to over medicate them either. The Frontline Plus already has more chemical in it that Frontline Top Spot. So I just give what is needed by their weight and divide the largest size available out and save more money that way also.
Sometimes I have to use Revolution topical since it kills ear mites and in the south they are more prevalent and depends too I think if the neighbors dogs have them across the fence. It is used for heartworm prevention also.
One other thing is Dr.s Foster and Smith have a book on medicines for your dog, it will give you the dosage of ivermectine needed to prevent heartworm, so you can dose that out each month by weight also if you are concerned about saving money on heartworm preventatives. This is one thing I wish the Heartworm Society of America on their website would promote, but alas Heartguard/Merial and others etc. pays them too well to not do it I am afraid. I wrote them about this years ago and they would not change their position on it.
The last thing is this <She has had several skin scrapings and they were always negative for parasites.> You would want to check the skin for bacteria that could be causing the problem also? To see which antibiotic would be effective for her skin infection.
In all I have written in total, these are just suggestions, pass them by your vet, (I am not a vet), but just giving suggestions of other alternatives.
