cost of concrete pad for garage

anyone who recently put in a concrete pad for a house or garage,,, can you
give me an approx price you paid?

I was given a quote for $15/ sq ft  
plus the cost of gravel and the termite teatment.

My site is ready to go, does not need any excavation, there is already a level pad with compacked rock.
the guy told me he would just put fresh gravel down and no need to dig.
He said that was better becasue you never know what you hit when you start digging in Puna....

Sounds kinda right. Assuming a 6" thick slab, 1000sq/ft puts you at 18.5 yards, and last time I checked it was going for around $260/yd for 3000psi delivered. Puts you right at about $5000 for the concrete alone. Steel probably another $4000, then the rest is the labor for formwork and the pour.

If the slab is super simple, you can save yourself some money by doing the formwork yourself, and laying the steel. It isn't rocket science. Then hire some guys who know a thing or two for about 3 hours to spread the concrete and finish it.

But what's a few thousand bucks for the piece of mind of letting the pros do the whole job?

update

I ended up hiring a concrete guy, it was flat rate of $12,000 , only thing else was I paid for the rebar and plastic.
He brought his own forms and handled all the ordering and dilivery and paying of the gravel, concrete , pump truck and his guys...

The slab was 24' x 40"  
so about $13.50 / sq ft installed....
