The NTSB (National Transportation Safety Board) website database contains a "brief" report of a Fairchild Hiller helicopter accident on April 26th, 1986. The report, rather the Safety Board's synopsis and determination of probable cause, can be read at the following link:

Unfortunately, the NTSB website's factual report contains no written narrative of the investigation. The investigation was conducted out of the NTSB's Los Angeles Field office and there is no precise location specified other than Keaau (a weakness in the data collection procedures at the time).
