My interpretation may not be correct but is based on my experience. The number he gave is an "incident number" or in their terms a "ticket number", as opposed to a "case number". Every call may be assigned an incident number but only incidents resulting in an actual "report" are assigned a case number. It could be that all case numbers generated during a shift require that a report be completed by the end of the shift. He may have delayed requesting the case number pending the receipt of your video and was planning pulling a case number (based on the incident number) if he was able to gather enough evidence that "justified" a report, in his mind. Probably a bit of laziness involved.

As the person who initially gathered the "evidence" your information, including identifiers like DOB, are necessary to document the chain of custody and as you are now a witness. He needs that info.

I would burn the video onto a CD to give to the officer. Burn another for your records. No other personnel will want to burn the video to CD for the officer as they then become part of the chain of custody.
