Frequent Flyer Miles

I'm looking to get a CC that will allow me to acquire air miles. Currently I'm with BofH and very happy with them. Before I check with them for what they have available I wanted to ask the question, who offers the best deal on miles per dollar spent and what I might expect for the annual fees. Are the fees minor compared to the benefits acquiring the miles? Do higher fees usually equate to more miles on the dollar?

Sorry for all the questions but I've never gone this route before and it is all new to me.
