I posted this in the events forum, but I hope it's ok to spread the word here, too.
I am a volunteer lay leader for this program. Since the support helped me so much, I am trying to get a group started for other folks in Puna, and we need a few more participants to join us.

Chronic Disease Self-Management Program

Topics include: goal setting and action plans, problem solving, fitness and exercise, medication management, effective communication, health eating, how to relax, evaluating new treatments

6 wk workshop for anyone with an ongoing health condition or their caregivers.

When: Wednesday afternoons, 1-3:30, from 1/16/13-2/22/13
Where: 14th and Paradise in HPP, UUs of Puna (turn R on 14th, white house on L)
Cost: No Fee!

Sponsored by the State of Hawaii and Hawaii County Office of Aging, preference to those age 60+, but not limited

Is this a religious based program, or strictly science/medicine based education? And what is the UUs on 14th?

Hi Carol,

No, it is not religious, it is an evidence based self- management education and support program coming out of Stanford university. We are holding it in the Unitarian Universalist fellowship hall.

This workshop was postponed until next Wednesday, 1/23 so that we can continue to register more participants. Please give me a call if you would like to join!
