Agree there should be more information available on incidents like this.
Some of this falls back to the Electrician. I wouldn't want this on my conscience. I think from now on I will refuse to energize any system until inspections are done.
One Thing I can always be sure of is that things will never go as expected.

As one sparky to another, let me know if you find out what happened. Inspection is too vague and there are too may unknown circumstances. Was this the first time it had been energized or was the pump operational and had some problem? Did the victim do the installation? What were the voltages and the size of the pump, etc.

Today's local fishwrap IDs the victim but says nothing about the circumstances.

This was an industrial accident involving some components that handle high voltage. I am not at liberty to divulge the information I have recently received. I Have to admit that the Malfunctioning components are a bit over my head. It's not something that I normally deal with at my job.
Even normal household voltages can kill. I have a protocol that I always follow. Disconnect power at the source. Test for the presence of voltage on the load side...test again before touching anything. Stay safe everyone. Aloha, KW

I see that there are a lot of extra opportunities to electrocute oneself on Big Island. The various systems of alternative power combined with utility power are basically ups(uninterrupted power supply) systems where it is easy to believe you have shut off the power when it is still hot just coming from another direction.
O.K. Call me slow but have to ask K.imo W.ires ...
Is your name here a tie in to acronym of the stuff you work with ?
