When I bought 5 yards of standard 3000psi concrete from West Hawaii Concrete, it was just a hair over $200/yard. No extra delivery, even all the way in Kau. The reinforcement was about $300. It was about $4.15 total material cost per square foot. I was able to do the formwork and reinforcement laying myself, with no prior experience, and it turned out well (and level).
I hired a couple extra guys to help with the pour, as I knew nothing, and had zero skill with the touchy parts (screed board and bull float).
As Rob said before, if it's gonna take multiple pours, which it almost certainly will, you gotta be darned sure you time the trucks right. I think West Hawaii Concrete quotes 7 minutes/yard pour time, and charge $1/min after that. But really, it's about getting the concrete to bind together. You don't want half of it setting before the next half comes down. In my case, it seemed like it was setting REAL FAST. It had already been in the truck ~70 minutes when we started pouring.
