You are probably right.  I'm not familiar with the Hawaii form for this purpose.  Some states require applicants to be a bit more specific, or more creative with their lying.

How many religions specifically tell their followers that vaccines aren't allowed? They would have had to start after vaccines were invented. I can only think of one such religion that began after that date; some think it's a cult.

There are very few, but a religion would not have had to start after vaccines were invented. The religion's leaders most likely interpreted the belief system in some fashion that says "vaccines bad".

If we could genetically engineer away low IQ and sociopathy, religion would die of natural causes. Perhaps there should be huge fines for parents of children who suffer because of their ignorance.

I’m not sure what the post about the minister-baby doctor is suggesting. Wasn’t this thread generally about vaccines? When I see my GP he refers me to a specialist. For instance, if blood work shows a kidney problem, he’d refer me to a kidney doctor not a pediatrician. The minister-baby doctor is probably good at his specialty but most likely has had little training in how vaccines work or how viruses spread through the population.

A doctor in one field is not necessarily qualified in every field of medicine. Even in the ministry a Protestant minister is not considered qualified to preach in a Catholic Church.

Pediatricians administer more vaccines than any other specialist - babies and toddlers get all sorts of vaccines so he had plenty of training.
