I could get an official answer from the County but I thought this might be faster and easier:
What is the rule regarding the square footage of a bathroom and the size of the window before it is necessary to install an exhaust fan?

The County of Hawaii uses the 1991 Uniform Building Code as amended by Hawaii County. There should be a copy of it at the Hilo Library if it isn't online somewhere.
As long as you have an exterior window you may not need an exhaust fan at all. If I remember (and it's been awhile since I've looked it up so you'd probably want to check this) the exterior window is supposed to be ten percent of the square footage of the floor area in "glazed exterior openings" (glass area)and five percent of the square footage of the floor area is to be in "openable openings".

Many people will put a 4016 over a shower in a kit house 5x8' bath and it just makes it. (for you really old timers, you might remember this referred to as a GI bath.)

Bathrooms, water closet compartments, laundry rooms, and similar rooms shall be provided with natural ventilation by means of openable exterior openings with an area not less than 5% of the floor area of such rooms and with a minimum of 1½ square feet. In lieu of required exterior openings, a mechanical ventilating system connected to the outside and capable of five (5) air changes per hour may be provided. The point of discharge of exhaust air shall be at least three feet (3’) from any opening into the building. [Section 1205©]

Thanks Wyatt, for looking that up. I was too lazy to go get the UBC from the bookshelf and hunt through it. Too bad I can't send you the 300 questions like that when drawing up a set of blueprints for a non-standard house. Having to find out these sorts of details each time a house deviates from "standard" is probably why so many houses look alike, IMHO.
