Building a stand alone refrigerator wouldn't require a permit unless you direct wired it to the wall. Building a refrigerated room would probably technically require a permit. Not that I've ever looked that up, though, I could be entirely incorrect on this but this is how it would seem to be to me.
When I lived on a small sailboat in the harbor we had no power so many folks had ice boxes instead of refrigerators. My friend who lived three boats over built one that would stay cold for over two weeks on one 18 pound block of ice. He really insulated it well, had the opening at the top and had a water loop in the drain so the cold air didn't leak out. It was mostly foam panels, some sort of reflective space wrap and fiberglassed and then painted blindlingly white since it was also outside in the cockpit which had sun during parts of the day. He would only open it once or twice a day, though.
