"terracore: the severe form (dengue hemorrhagic fever) that can occasionally be fatal is caused only by getting it twice. There are four strains of dengue; if you get one, you are then immune against that one. But if you later get infected with a different strain, it develops into DHF due to the off-kilter immune response."
Unfortunately, since the first infection can be confused with something else, it's impossible to know if you are getting a first or second infection (granted in Hawaii with the disease not being endemic, this is not a concern for most of us). The science behind this second infection being worse is fascinating: "The reasons for variation among patients infected with the same virus are unknown but stem from a complex system of virus-host interactions. Dengue fever becomes more virulent during a second infection by means of antibody dependent enhancement...Antibody-dependent enhancement (ADE) occurs when non-neutralising antiviral proteins facilitate virus entry into host cells, leading to increased infectivity in the cells. Some cells do not have the usual receptors on their surfaces that viruses use to gain entry. The antiviral proteins (i.e., the antibodies) bind to antibody Fc receptors that some of these cells have in the plasma membrane. The viruses bind to the antigen binding site at the other end of the antibody. ADE is common in cells cultured in the laboratory, but rarely occurs in vivo except for dengue virus. This virus can use this mechanism to infect human macrophages, causing a normally mild viral infection to become life-threatening...The phenomenon of ADE may be observed when a person who has previously been infected with one serotype of DENV becomes infected many months or years later with a different serotype. In such cases, the clinical course of the disease is more severe, and these people have higher viremia compared with those in whom ADE has not occurred. This explains the observation that while primary (first) infections cause mostly minor disease (DF) in children, secondary infection (re-infection at a later date) is more likely to be associated with severe disease (DHF and/or DSS) in both children and adults."

Per telephone call today to Governor's office, there will be a Press Conference in Hilo on Monday. They are also considering "talks" in Puna.
Also spoke with DOH, and they urge us all to contact our Reps and Senators, and implore them to act on this outbreak before it becomes endemic. DOH simply has no funding to do in depth testing of mosquitos and spraying infested areas. This is one can that simply cannot be "kicked down the road" or our health, State economy, and life as we know it will never be the same. It's time to let the powers that be know that we are watching and waiting for them to act! Please make some calls.
