From the Hawaii Quarantine website:
"Direct Release on Neighbor Islands: Pets may now qualify for direct release at Kona Airport (Big Island), Kahului Airport (Maui) and Lihue Airport (Kauai)"
http://hdoa.hawaii.gov/ai/files/2013/01/...d-2013.pdf

Yes, they sure do have a lot of contradictory stuff. Planning on bringing a cat over this year. Not looking forward to it. If you don't do everything EXACTLY right, it gets real bad.
Here's a pretty good description of what it's like:

I found the release officer for Kauai online and he was really nice and helpful (even though you're coming to Big Island). You can call him with questions.

As I said, this is something you absolutely have to do correctly or it's going to be hell for you and the dog.
For me, everything else involved with moving from the mainland seems easy compared to this!!!

Just know that if you fly non-stop to Kona with your pet (as I did, it worked very well), you need to get your neighbor island release form, and you have to arrange for a vet to meet you at the airport to do a final inspection/review of your pet. I think they're really looking for ticks, and other creatures that could travel with your best friend.
As others have said, follow the directions to the "t", and make sure you read the neighbor island release information - that's slightly different from going through Honolulu.
On a side note - we flew Alaska Airlines from SJC to KOA. They were wonderful with our pet. Several of our neighbors have flown them as well - and everyone has had nothing but positive reviews. Be sure to call and reserve your pets place on the plane - they only accept one or two kennels on each flight.

Coastal,
Were you able to fly the pets in the cabin?
Great feed back, and an excellent read from that blog. Mahalos !

Check the airline's website for rules re: flying pets the cabin, but the easy answer is that they usually fly with the luggage. There is a long thread on Punaweb about flying pets into Kona - do a search for it...lots of good info there.
